Opioid Health Homes (OHH) are Medicaid funded programs that focus on care coordination, specifically for individuals with Opioid Use Disorders (OUD). While not treatment specific, OHH services were designed to help beneficiaries connect to medically necessary services and to address the complexity of comorbid physical and behavioral health conditions.
Opioid Health Homes are not residential programs. Participation in OHH is voluntary and individuals enrolled in an OHH can opt out at any time. To qualify for an OHH, individuals must:
- Have an Opioid Use Disorder (OUD)
- Be a Medicaid beneficiary (there are limited funds for individuals without Medicaid)
- Be a resident of Lenawee, Livingston, Monroe or Washtenaw Counties
Once enrolled, individuals work with a health care team to coordinate, support, and help manage their treatment, recovery and other health care and social needs. OHH is there to improve care coordination for physical health, Medication Assisted Treatment and other needs such as transportation, housing, etc.
CMHPSM funds five regional providers to implement Opioid Health Homes, Packard Health, Family Medical Center of Michigan, Ann Arbor Comprehensive Treatment Center, Therapeutics, and Passion of Mind Healing Center.
