Humana: (2/17) – Humana launched Humana Care Support Pilot to help Medicare Advantage members to better manage their chronic conditions. Humana Care Support pilot will help Humana Medicare Advantage members and others live independently in their homes, where they feel confident, safe and secure. “Long before the pandemic, the home has become the place where many older adults living with multiple chronic disease want to receive care and to age in place,” said William Fleming, PharmD, Segment President, Clinical and Pharmacy Solutions for Humana. “Our Humana Care Support pilot will be instrumental in using best practices in coordinated home health, care management visits and virtual visits to help our members improve their health,” he said.
Signify Health: (2/16) – Signify Health a value-based care platform that uses advanced analytics and other technology to shift health services toward the home, announced the closing of its initial public offering. Signify Health received net proceeds of approximately $609.7 million.
Business Wire: (2/16) – SCAN Group has made a strategic investment in Monogram Health. Monogram Health is a leading kidney care management company providing at-home care and data-driven solutions for patients living with chronic kidney and end-stage renal disease.
Inside Health Policy: (2/11) – Advocates are glad to see home care included in the Energy & Commerce COVID-19 relief bill. “I think this will help in supporting states to think about what, of the many changes that they've made… should stay, what needs further investment," Kitty Purington, senior program director at the National Academy for State Health Policy said. Amber Christ, a directing attorney with Justice in Aging noted that “These funds are absolutely critical in helping people transition out of nursing homes and other congregate care settings.” A LeadingAge spokesperson told Inside Health Policy that the 7.35% increase is a step in the right direction, but the group will continue to push for additional provider relief funding.

The at-home focus helped Baystate Health save $1,729 per visit in risk-based population.
Home Health Care News: (2/10) – Two in-home care providers — ConcertoHealth and Perfect Health — announced Wednesday that their organizations have merged. Together, the companies will operate under the name “ConcertoCare.” ConcertoCare will offer in-home primary care, virtual primary care and Program of All-Inclusive Care for the Elderly (PACE) services to dual-eligible seniors and those enrolled under Medicare Advantage.
Home Health Care News: (2/7) – Home-based hospital care has been a hot topic ever since the Centers for Medicare & Medicaid Services (CMS) unveiled its “Acute Hospital Care at Home” initiative in November. Mount Sinai Health System launched Mount Sinai at Home in 2014 as part of its offerings, with the help of a three-year CMS Innovation Center grant. Now, Mount Sinai has paired with Boston-based Current Health — a remote patient monitoring platform — to help keep an eye on cancer patients at their homes.
Inside Indiana Business: (2/3) – Central Indiana nonprofit CICOA Aging & In-Home Solutions is launching a tech startup designed to match people who need in-home care with providers who offer the services. The nonprofit says Duett empowers older adults and people of any age with intellectual, developmental or medical disabilities to remain comfortably and safely in their homes and out of institutional care.
