Over 85 care providers and organizations are using the e-MOLST web portal to engage patients and their legal advocates in a high quality serious illness conversations. The new web portal provides conversation guides to gain a shared understand of patient goals & choices, an e-MOLST form to document patient choices, and a link to a MA Health Care Proxy in 10 languages. Patients can use their smart devices or email address to digitally sign the e-MOLST form.
