Since the Public Health Emergency (PHE) will be ending, it is very important to know what to do to be sure your Medicaid coverage continues if you are still eligible. This applies to everyone who receives Medicaid, even those who receive HCBS services of any kind.
Update your address. If you have moved within the last three years, be sure that Medicaid has your correct address.

who is currently covered by Medicaid will to go through the renewal (redetermination) process.
In order to ensure you receive information from Medicaid they must have your current address. You may lose your coverage if Medicaid cannot reach you.
How to be sure Medicaid has your current address: Your address can be updated online, by mail, by fax or by going into your local county Medicaid office in person. To update your address online go to: CO.gov/PEAK. The Adams County Medicaid office is located at: 11860 Pecos Street, Westminster, CO 80234
Phone: (720) 523-2000
Fax: (303) 227-2380
Also – Notice of Action: Some people will be automatically renewed for Medicaid based on information from other sources. In this case, you will receive a “Notice of Action” letter from Medicaid instead of a renewal packet. If you receive a “Notice of Action” this means you will still have Medicaid and don’t need to do anything.
